Before start provisioning Polycom phones from the Elastix GUI remove the microbrowser (5lines) section from following files.
/usr/share/elastix/endpoint-classes/tpl/Polycom_global_sip_1.tpl
/usr/share/elastix/endpoint-classes/tpl/Polycom_global_sip_2.tpl
